In addition to the above personal information, we also collect information about your serious illness, age, gender, ethnicity, date of birth, and electronic signature at the time of your video application. Video application forms as well as our healthcare referral form also collect personal information of name, phone number, and email for a "secondary contact" (ie family member or friend) who may be involved with helping arrange your video filming or who may receive your final custom edited video at completion. The information submitted through our video application forms as well as our healthcare referral form is seen only by the key organizational members of Amadora responsible for facilitating logistics of the scheduling and filming production. Our video production team communicates with video applicants and their chosen family members, or healthcare referral partners through both email and text, as deemed appropriate.
