Exclusively available to individuals with stage IV cancer, ALS, Huntington’s, Parkinson’s, early-stage Alzheimer’s, other life-limiting illnesses, and those actively enrolled in hospice care.

Who Can Be Referred
Patients diagnosed with a life-limiting illness, such as stage IV cancer, ALS, Huntington’s, Parkinson’s, early-stage Alzheimer’s, other life-limiting illnesses, and those actively enrolled in hospice care.
Patients must consent to participation prior to referral submission.

No. Thanks to our generous donors, there are no costs to your patients or to your organization.
Once a referral is sent to us, we take it from there. There is no extra work or burden to your staff.
Once a referral is received, it takes us a few days to verify their eligibility and reach out by phone or email to the individual or chosen family member.
We can quickly mobilize when time is of the essence. Our team has completed recordings at home, in hospice, and in hospitals with as little as 24 hours' notice. To help prioritize appropriately, please indicate the time-sensitive urgency on the referral form. Clear timing information allows us to fast-track scheduling and coordinate with patients and families as quickly as possible.
Yes. Given the emotional preparedness involved in recording a legacy video, the patient must have prior knowledge and agree to record a video prior to submitting a referral.

Typically, it takes about one week to verify eligibility, schedule the first pre-filming call, and lock in a filming date. If you have a patient under a critical timeline, please notify us, so we can expedite the process as quickly as possible.

The day of filming takes 60-90 minutes if it is scheduled during one of our quarterly Share Your Story events or it takes around 3 hours total for an in-home filming session.

We have found that most individuals prefer to film on their own so they feel more comfortable saying everything they want. However, if the patient prefers filming with a loved one present, we can accommodate one family member.
We take the patient’s lead throughout all parts of our process. We adjust filming time to their stamina level.
From filming to finished video, it takes approximately eight (8) weeks.
The patient is free to share their video with anyone they wish. There is only one exception: the video cannot be used for any commercial purposes in which the participant, family member, or acquaintance can profit.
We take privacy seriously. All Legacy Coaches and videographers sign agreements of confidentiality that prevent the sharing of participant names, details, or video clips with anyone outside of those involved directly in the process. All videos are shared through a private, unsearchable link on a secure video platform. We do not publicly share anyone’s full-length video, though all participants consent to allowing Amadora Legacy Project to share brief video clips in Amadora’s owned materials and channels (website, brochures, social media) for the sole purpose of raising awareness and continued support of the charity’s mission.
